Multiple-entry visa for Chinese citizens residing in Canada

Requirements(Please do not staple on the documents)
1. The completed VISA Application form to enter Japan, Please fill out form entirely before visiting our office. https://www.mofa.go.jp/files/000124525.pdf For handwriting, please do not use any erasable pen.
2. One 45mm x 35mm passport-type photos taken within the previous six months (should have a date stamp on the back of the photo)
(if the photo does not have the date stamp, you may be asked to take another photo or provide the receipt when the photo was taken)
3. Valid passport and a photocopy of status page of this passport
4. Both original and one photocopy of your Canadian Immigration document (ie PR Card or Work Permit and Visa, Study Permit and Visa which is valid longer than three years from date of your Japanese visa application)
5. Last six(6) months bank statements, it must be from Canadian financial institutions and it must include an account holder's full name, issuing date and transaction history)
6. Most recent(Tax year 2023) of the Notice of Assessment issued by Canada Revenue agency or T4
7. A letter stating the reason why you would like to apply for the multiple-entry visa
8. A schedule of what you are planning to do in Japan (including specific dates and places day by day)
9. Flight itinerary issued by travel agency or airline company. The itinerary must cover your entire trip starting from Canada and ending in Canada.
If you are going to a third country from Japan, please provide a photocopy of the visa of the country.
10. Visa fee(in cash $58/per person)
11. Self-addressed registered return envelope (only required if you wish to receive your passport by mail)
Additional document for applicant who provide spouse or parent's bank statement and yours: Address proof of yourself and spouse or parent'(i.e. Driver's licence)

For family members of the main applicant, please provide a document proving your kinship(Marriage certificate, Birth certificate etc.) and a photo copy of the Japanese visa of the main applicant(if the family member(s) does not apply with the main applicant).
